KOVAR Grants

Grant requests for financial assistance submitted to KOVAR must contain:

• Name, address and background of requesting organization including a description of the client population and the number and percentage that are mentally retarded. A copy of the organizations's budget must be included.
• Name, address and phone numbers of requesting official and contact person.
• Detailed write-up on goods and/or services needed and how these goods and/or services will benefit persons with mental retardation. Describe impact on program if grant is denied or partially funded. Include prices from 3 sources for all merchandise requested.
• Evidence that organization has IRS 501 (c)(3) tax exempt status.
• Information on all grants previously awarded by KOVAR (include dates and amounts).
• Authorization to display a permanent plaque at site identifying an Honoree for the grant request.
• Indication of where the organization learned of KOVAR.
