In order for your child to receive his/her First Reconciliation and First Eucharist, you and your child must fulfill the Diocesan requirements listed below for sacramental preparation. By registering your child and checking the boxes below, please note that registration does NOT guarantee the reception of the First Sacraments.

If you and your child are not able to commit to the Diocesan requirements and tentative schedule as below, please delay preparation to another year when it is more conducive to your and his/her schedules. If requirements are not met, the reception of the sacrament may be delayed at the discretion of the Pastor and Christian Formation Coordinator.

PAY the one-time registration fee for both sacraments before the scheduled due date. This fee covers the curricular programs for the sacraments as well as supplies needed for the retreats and the cost of the First Eucharist Banner. Please select this field.

SUBMIT a scanned or paper copy of my child's Baptismal certificate to the Christian Formation Coordinator NO LATER THAN 1 month prior to First Reconciliation, unless my child was baptized at Church of the Holy Family. Please select this field.
